PLEASE DO NOT SCHEDULE AN APPOINTMENT UNTIL YOU HAVE READ THE FOLLOWING:

• A completed Application, form DS-11, available at
• Proof of U.S. citizenship – an expired passport (no photocopy necessary), original or certified copy of U.S. Birth Certificate, Certificate of Birth Abroad, or Certificate of Naturalization and a photo copy of the document.
• Photo Identification – Driver’s License, Government or State-issued ID card and a photo copy of the front and back of the card.
• Application Fee ($130.00) – Payable to U.S. Department of State by check or money order only.
• Execution Fee ($35.00) – Payable to 91¾ÅÉ«ÊÓƵ.
• Passports for Minors (Under 16 years of age) - Both parents, along with the child, must be present with photo ID.

Already Have A Passport? – If you already have a passport, you may be eligible to renew it by mail.
